Polarean Declaration of Compliance

Per California Health and Safety code, section 119402, Polarean is providing a written declaration of
compliance to describe its Compliance Program as follows:
Polarean is committed to encouraging and maintaining a state of compliance and ethical behavior for its
employees and representatives and adheres to guidelines outlined in “Code on Interactions with Health
Care Professionals” (aka the “PhRMA Code” published 2022-01-01), the “AdvaMed Code of Ethics”
(published 2022-06-01), and the “OIG Compliance Program Guidance for Pharmaceutical
Manufacturers” (published 2003-4-18). In addition, internal policies outline details regarding Polarean’s
compliance program and offer guidance to employees and representatives such as limits on spending or
gifts to health care professionals. Polarean declares that it is conforming to the codes and guidance
listed above, including the California code, as well as with its own internal policies and procedures.
